PT Wound Care Nurse Practitioner

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to provide specialized wound services to patients residing in skilled nursing facilities. In this role, you will work with an interdisciplinary care team to deliver comprehensive wound management to the elderly population in long-term care settings.

Responsibilities

• Provide wound care services for long-term care patients residing in skilled nursing facilities
• Prescribe appropriate medications, including controlled substances in accordance with Ohio law
• Order, review, and follow up with treatments and diagnostic testing
• Communicate regularly with nursing home staff to discuss changes in patient condition and care plans
• Collaborate with primary care physicians on patient care coordination

Clinical Setting

You will work in skilled nursing facilities providing medical services to residents of long-term care facilities.

Required Qualifications

• Current, unrestricted Nurse Practitioner license in the state of Ohio
• Board certification
• Clinical background in adult, family, or geriatric specialties
• Clinical background in wound care, including chronic, debilitating, stalled, or infected wounds
• Comfort with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy ulcers
• Ability to stage pressure injuries according to NPIAP standards
• Familiarity with wound care best practices and advanced wound care products including collagens, alginates, foams, and silvers
• Experience with wound vacs and other durable medical equipment such as pressure relieving or reducing devices
• Experience working in a long-term care setting
• Ability to work with an electronic medical record (EMR)
• Ability to work within an interdisciplinary care team to coordinate patient care

Preferred Qualifications

• Wound care certification or willingness to obtain certification
• DEA licensure
