Northgate Park is seeking a Part-Time Licensed Practical Nurse (LPN) to serve as the shift supervisor and team leader in our senior living community. In this role, you will coordinate resident care, supervise Resident Care Assistants and Medication Technicians, and oversee the overall functioning of the Wellness Department during your shift. You will report to the Wellness Director and may make residence decisions in the absence of the Executive Director and Wellness Director.
Clinical Setting
This position is based in an assisted living community serving seniors. Experience with dementia care is preferred.
Responsibilities
- Plan and delegate work assignments to ensure effective shift coverage and coordinate resident care
- Participate in and supervise the delivery of personal care to residents
- Implement service plans and collect data regarding resident health status
- Act as team leader for your shift, coaching and counseling staff regarding care practices and standards
- Follow proper disciplinary procedures and write disciplinary reports when necessary
- Maintain professional demeanor when interacting with residents and families
- Use tactful, diplomatic communication in sensitive or emotionally charged situations
- Report any suspected or witnessed instances of verbal, mental, or physical abuse to the Wellness Director and take appropriate action
- Administer medications according to physician orders on the medication administration record (MAR)
- Document medications administered on the MAR in accordance with company policy
- Check MARs immediately after administration and prior to end of shift to ensure documentation is complete and accurate
- Conduct change of shift reports with oncoming and outgoing shift personnel
- Maintain security of medication cart and narcotic keys during your entire shift
- Conduct narcotic counts at the beginning and end of every shift in accordance with company policies
- Transcribe physician orders, review MARs for accuracy, and notify pharmacy of changes
- Respond to emergencies in a prompt, calm manner
- Orient and assist with on-the-job training of new associates and participate in evaluation of their knowledge, skills, and abilities
- Monitor e-call system equipment to ensure proper working condition and timely caregiver response
- Monitor and maintain resident charts
- Report significant changes in resident condition to the physician and responsible party
- Document all observations, follow-through, and new orders or instructions from the physician
- Understand and use Centers for Disease Control and Prevention Standard Precautions and OSHA's Occupational Exposure to Bloodborne Pathogens
- Operate designated medical equipment, copy, scan, and fax machines, and telephone
- Document and report resident incidents as required by state regulations
- Participate in and support the resident-centered activities program
- Function as a team member in sharing responsibilities for resident services
- Keep the Wellness Director informed of all changes in resident health and any problems within your shift
- Participate in associate meetings and other required meetings
- Perform other duties as assigned by the Wellness Director
Required Qualifications
- Graduate of an accredited school of nursing
- Active state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
- Minimum of two years of nursing experience
- Ability to read, write, and speak in English
- Good physical health and free of communicable diseases
- Ability to lift 40 pounds
- Lifting, bending, and pushing medication cart throughout the residence required
- Completion of drug testing and criminal background check upon hire and upon request of supervisor

- Experience in an assisted living setting
- Dementia care experience
Compensation and Benefits
Pay: $30.00 per hour
