Psychiatric Registered Nurse - Behavioral RN - RN - Home Health - PRN

This is a PRN (as-needed) psychiatric nursing position providing home health care to patients of all ages. You will administer nursing care in clients' homes, coordinate with interdisciplinary teams, families, and referring agencies, and support clients in recovery, maintenance of life functions, and prevention of illness relapse.

Responsibilities

• Provide direct nursing care to psychiatric patients in their home environment
• Develop, initiate, and revise Plans of Care as necessary to ensure quality and continuity of care
• Regularly re-evaluate clients' nursing needs and evaluate outcomes of care
• Initiate appropriate preventative and rehabilitative nursing procedures and plan for discharge from services
• Provide effective communication with clients, families, team members, and other healthcare professionals
• Participate regularly in case conferences
• Apply infection control and universal precaution concepts to protect both clients and staff
• Coordinate services and inform physicians and other personnel of changes in clients' conditions and needs
• Monitor assigned cases to ensure compliance with third-party payer requirements
• Complete documentation in a timely manner in compliance with company policy
• Supervise, teach, and provide clinical direction to other nursing personnel including LVNs, LPNs, HHAs, and CNAs
• Perform on-call responsibilities and provide on-call services to clients and families as assigned
• Participate in in-service programs and maintain professional competency
• Meet mandatory continuing education requirements of the agency and licensing board

Required Qualifications

• Two years of experience as an RN in a psychiatric clinical care setting
• Current state professional RN license in any applicable states
• Current CPR certification
• Current driver's license in good standing
• Completion of an accredited Registered Nursing Program
• Ability to read, write, and comprehend English
• Hepatitis profile if required to make visits

Preferred Qualifications

• Home health nursing experience
• Psychiatric-Mental Health Nursing Certification
• Bachelor of Science in Nursing (BSN)

• Ability to lift and transfer up to 50 pounds
• Hand-eye coordination and manual dexterity
• Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions
• Ability to meet patient and family individualized psychosocial needs
