Eventus WholeHealth is seeking a Psychiatric Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide behavioral health and psychiatric medication management services within skilled nursing and senior living communities. In this role, you will conduct routine patient rounding, evaluate and manage mental health conditions, and collaborate closely with interdisciplinary teams to support residents' emotional and cognitive well-being. You will work with facility partners to ensure continuity of care, optimize treatment plans, and promote a therapeutic environment using a value-based approach that emphasizes proactive care and improved quality outcomes.

Responsibilities

  • Provide behavioral health and psychiatric medication management services to residents in skilled nursing and assisted living facilities
  • Conduct routine patient rounding and ongoing evaluation of mental health conditions
  • Develop and optimize individualized treatment plans
  • Collaborate with physicians, behavioral health specialists, care coordinators, and facility staff to create integrated, person-centered care plans
  • Support residents' emotional and cognitive well-being
  • Promote a therapeutic environment within facilities
  • Work with facility partners to ensure continuity of care

Required Qualifications

  • Current NP or PA licensure in the state of practice
  • Previous professional patient care experience
  • Exceptional patient-centered service skills, including compassionate communication, active listening, and professionalism
  • Ability to build trusting relationships with residents, families, and facility staff
  • Strong collaboration skills and ability to work effectively across interdisciplinary teams

Preferred Qualifications

  • Experience in geriatrics, family medicine, or psychiatry
