Psychiatric Nurse Practitioner

TCA Health, a Federally Qualified Health Center on Chicago's far south side, is seeking a Psychiatric Nurse Practitioner to join the Behavioral Health Department. This position plays a key role in our fast-growing behavioral health services as we transition to an integrated care model. You will provide direct clinical care to patients while supporting primary care providers and care managers in treating patients with behavioral health problems. This is a fast-paced environment where patient care is the primary focus.

Clinical Setting

You will work in a multi-disciplinary primary care community health center serving underserved communities on Chicago's south side. The role includes on-site patient care and outreach for our School Based Health Clinic and Mobile Health Unit. You will work with children, adolescents, and adults.

Responsibilities

• Perform initial and ongoing clinical evaluations
• Manage medication treatment and develop treatment plans
• Perform crisis intervention by telephone for patients during non-office hours as assigned
• Consult and advise clinical staff and the Clinical Director
• Manage care coordination with other appropriate care providers
• Enter data directly into the electronic medical records system
• Perform and assist in the administration of routine diagnostic and laboratory studies, clinical and therapeutic procedures
• Develop presumptive nursing diagnoses and implement treatment plans based on advanced nursing assessment consistent with established practice guidelines
• Document assessments of patients' health and illness status
• Evaluate patient responses to treatment plans and redirect care based on findings
• Assess patient clinical and health education needs; instruct and counsel patients in one-on-one and small group settings regarding health matters
• Refer patients to clinical and health education classes as appropriate
• Provide telephone care to assigned patients as necessary
• Obtain assigned patient vital signs and provide documentation
• Act as a resource person to other staff members

Required Qualifications

• Master's Degree in Psychiatric Mental Health Nurse Practitioner from an accredited program
• Current Illinois RN license
• 3 or more years of clinical experience with demonstrable experience in diagnosis
• CPR Certification
• Team player with excellent interpersonal skills

Working Conditions

This position is in a clinical setting with high likelihood of blood or body fluid exposure under OSHA standards.

Compensation

Salary: $100,000 - $125,000

Employment Status

Exempt, full-time position reporting to the Chief Medical Officer.
