Psychiatric Nurse Practitioner

Greater Mental Health of New York seeks a Psychiatric Nurse Practitioner to join the Youth ACT-Westchester program, serving children and youth with Serious Emotional Disturbance (SED) throughout Westchester and Rockland counties. This mobile hybrid role provides intensive, coordinated psychiatric and medical services to youth returning home from inpatient or residential settings, at risk of entering such settings, or who have not adequately engaged in traditional community-based services. You will deliver both telehealth and face-to-face appointments in clients' homes, communities, and agency offices, working as part of an integrated multidisciplinary team.

Clinical Setting

Youth ACT is an assertive community treatment program delivering evidence-based, trauma-informed, person-centered services to children and families throughout the Lower Hudson Valley region. The program emphasizes recovery, self-determination, choice, and shared decision-making.

Responsibilities

• Act as prescriber for all Youth ACT clients
• Conduct psychiatric assessments via telehealth, office visits, and in-home/community settings, including psychiatric history, course of illness, response to treatment, mental status examination, and diagnoses
• Present assessment results at team meetings and treatment planning meetings
• Collaborate with the team in assessing clients' physical health, making appropriate referrals to community physicians, and coordinating medical and psychiatric treatments
• Prescribe psychotropic medications and conduct regular assessments of therapeutic responses and side effects
• Educate clients regarding psychotropic medications
• Perform individual, group, and family supportive therapy and illness education
• Model and teach a person-centered approach, honoring choice, self-determination, and shared decision-making
• Establish and maintain a recovery-oriented culture grounded in service excellence
• Provide support, education, and counseling to family members to help them understand mental illness, collaborate in treatment, and support their family member's recovery
• Provide on-site crisis assessment and management of patients on-site, via telehealth, and over the phone
• Respond to the Team Leader (or designee) after hours for consultation in extenuating situations
• When possible, be actively involved in hospitalization of clients, including regular communication with attending psychiatrists, team members, and inpatient staff
• Submit written reports and oral testimony in commitment and other legal proceedings and for treatment, rehabilitative, and support services matters (e.g., Social Security applications, prior authorization of medication)
• Document client progress to maintain permanent records according to established methods and procedures
• Participate in morning team meetings and treatment planning review meetings
• Follow established Youth ACT policies and procedures, agency policies, and maintain appropriate professional standards
• Use individual client data to inform clinical decision-making and identify areas for quality improvement
• Share client data with clients to facilitate shared decision-making and keep them informed of their progress

Required Qualifications

• Current licensure as a Psychiatric Nurse Practitioner by the New York State Education Department, maintained in good standing throughout employment
• Current licensure by the New York State Education Department
• Current Drug Enforcement Administration (DEA) licensure
• Valid driver's license with a driving record satisfactory to the employer's insurance carrier
• Current New York State Defensive Driving certification
• Clearance through fingerprinting by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL)
• Clearance through the New York State Medicaid Exclusion List
• Clearance by the New York State Office of Children and Family Services for instances of child abuse and/or neglect
• Legal eligibility to work in the United States without sponsorship
• Basic computer knowledge
• Reliability and ability to work independently
• Ability to maintain confidentiality

Required Competencies

• Experience understanding family systems based on the diversity of cultural and ethnic backgrounds
• Experience and understanding of various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious emotional disturbance, and recovery
• Knowledge of Assertive Community Treatment, family support services, and community resources
• Use of Motivational Interviewing, recovery, and trauma-informed approaches when delivering services
• Highly organized and self-motivated
• Proficiency with computers
• Familiarity with and ability to use Electronic Health Record (EHR) systems
• Effective crisis intervention skills
• Ability to uphold fidelity to the Youth ACT model
• Ability to maintain timely and accurate documentation in accordance with agency policy
• Active participation in multidisciplinary teams and case conferences

Schedule and Employment

• Hours per week: As needed
• FLSA Status: Per Diem, Non-exempt
• Location: Tarrytown, NY and Rockland, NY (Mobile)
• Hours per week, reporting structure, and location are subject to change per program needs

Compensation

• Pay Range: $65-$100 per hour

Additional Requirements

• This is a Mandated Reporter position. Under New York State Justice Center legislation, anyone with regular and substantial contact with persons with special needs is defined as a "custodian" and therefore a mandated reporter. You can be held liable by both civil and criminal legal systems for intentionally failing to make a report.
• The Psychiatric Nurse Practitioner will be using agency vehicles and is responsible for adhering to Agency Driver Policy.
