Northern Rivers Family of Services is seeking a Psychiatric Nurse Practitioner to provide comprehensive psychiatric services to adults and youth enrolled in our Assertive Community Treatment (ACT) programs. This full-time position is based in our Queensbury office and requires community travel to provide on-site, community-based, and home-based services.
The ACT programs serve individuals diagnosed with serious mental illness, and this role is central to the clinical team's ability to deliver psychiatric care, medication management, and clinical consultation.
Responsibilities
- Provide comprehensive psychiatric services to youth and families enrolled in Youth ACT and adults enrolled in Adult ACT programs
- Prescribe medications and monitor and review drug regimens for enrolled individuals
- Conduct medication evaluations and reviews for ACT participants on-site, in the community, and in home settings
- Provide clinical expertise, consultation, and technical assistance to the Youth and Adult ACT teams
- Provide consultation to staff on diagnostic and treatment questions
- Participate in formal and informal training and education for staff
- Participate in quality assurance activities and consult with teams in developing and reviewing service plans for all enrolled individuals
- Collaborate with county and community partners
- Participate in quality audits and utilization reviews
Schedule
- Full-time, 40 hours per week
- Community travel required
Required Qualifications
- Minimum one year of experience working with youth and adults diagnosed with mental illness
- Active, full licensure as a Nurse Practitioner in the State of New York
- CPR Certification
- Ability to work independently
- National Provider Identification (NPI) number issued by the Centers for Medicare and Medicaid Services
- Federal Drug Enforcement Administration (DEA) number to prescribe controlled substances
- NYS Bureau of Narcotic Enforcement (NYS DOH BNE) registration to prescribe controlled substances
- NYS Medicaid Enrollment (when prescribing to NYS Medicaid-enrolled clients)
