Psychiatric Mental Health Nurse Practitioner - Salt Lake County

Gadzoom is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join the Psychiatry Department and serve patients throughout Salt Lake County, Utah. This role offers a flexible work schedule with opportunities for professional growth within a patient-centered care model that emphasizes continuous, integrated care and active patient engagement.

Clinical Setting

You will work within Gadzoom's integrated care model, collaborating with an interdisciplinary healthcare team to serve diverse patient populations across Salt Lake County. The role may require you to serve patients throughout the area, so candidates should live in or near Salt Lake County or be able to reliably commute.

Responsibilities

• Perform clinical assessment and appropriately manage patients
• Collaborate with interdisciplinary healthcare team members
• Order and interpret diagnostic tests as appropriate
• Perform procedures as trained and appropriately credentialed
• Complete accurate and timely progress notes and admission/transfer orders
• Educate patients on disease and treatment plans
• Report to the Director of Behavioral Health and assigned Medical Director

Required Qualifications

• Certified Psychiatric Mental Health Nurse Practitioner
• Licensed provider in Utah or ability to attain unrestricted state license

Preferred Qualifications

• Minimum 1 year of post-acute experience
