PRN - Nurse Practitioner

Four Pillars Hospice is seeking a PRN Nurse Practitioner to provide clinical oversight and palliative care to patients and families during end-of-life care. In this role, you will work collaboratively with an interdisciplinary team to develop and implement individualized plans of care, consult with attending physicians, and provide direct clinical assessment and education to patients, families, and staff.

Responsibilities

• Collaborate with the interdisciplinary team to ensure appropriate evaluation and palliative treatment of terminal disease
• Develop and consult on patient and family plans of care in collaboration with the interdisciplinary team
• Educate patients and families regarding disease process, self-care techniques, and end-of-life care; provide training to staff as needed
• Perform and document patient and family visits based on the plan of care and identified needs
• Interact with attending and consulting physicians regarding palliative care in collaboration with the hospice physician
• Provide clinical education to interdisciplinary team members on palliative care issues as needed
• Actively participate in interdisciplinary team meetings with effective and timely communication
• Inform physicians and other personnel of changes in patient needs and outcomes of interventions
• Complete required face-to-face patient encounters prior to the third benefit period and each period thereafter
• Provide clinical assessment results to the hospice physician following face-to-face encounters
• Maintain current knowledge in pharmacology and palliative medicine
• Provide on-call services to patients and families as assigned
• Adhere to standard precautions and all hospice policies, including those regarding protected health information

Required Qualifications

• Bachelor's degree from an accredited School of Nursing
• Master's or Doctoral degree from a Nurse Practitioner-focused nursing program
• Current state license as a Registered Nurse
• Current state license as an Advanced Practice Nurse
• Current driver's license
• Two years of experience as an Advanced Practice Nurse in a clinical care setting
• Comprehensive knowledge of pharmacology, palliative care, and end-of-life care
• Strong interpersonal skills
• Reliable transportation and valid auto liability insurance
• Ability to work a flexible schedule
• Ability to travel locally
• Ability to remain calm in stressful conditions and make quick clinical decisions

Preferred Qualifications

• Prior experience in hospice or home health settings

• PRN (as needed) position
• Flexible schedule required
• On-call services as assigned
