PRN Hospice Nurse Practitioner

Elara Caring is seeking a PRN Hospice Nurse Practitioner to provide high-quality palliative and hospice care to patients in their homes across the West Territory. In this role, you will work with an interdisciplinary team to deliver compassionate end-of-life care, manage symptom relief, and support patients and families during their final journey.

Responsibilities

• Complete routine and emergency face-to-face recertification visits with patients entering their third benefit period or beyond, including admissions, transfers, and extended length of stay cases
• Record observations, treatments, and pertinent clinical information in patient records
• Communicate findings and clinical recommendations with the Medical Director, attending physician, branch leadership, and interdisciplinary team
• Provide physical and emotional support to patients and families during periods of crisis
• Triage consults by patient acuity level
• Perform bedside palliative care with patient and family education
• Prevent unwanted hospitalizations by meeting patient and family needs in the home setting
• Prepare and maintain accurate patient records, charts, and documentation to support sound medical practice and appropriate reimbursement
• Complete accurate billing and coding for all patient encounters according to CMS guidelines
• Provide training and continuing education to staff
• Assist in development of clinical practice guidelines and standards supporting quality palliative care
• Maintain confidentiality regarding patients, families, staff, and the organization
• Perform other duties and projects as assigned

Required Qualifications

• Master's degree in nursing
• Current certification through a national Nurse Practitioner licensing board
• Current licensure as a Nurse Practitioner in the state of practice
• One or more years of clinical nursing experience
• Valid driver's license and insurance with reliable transportation
• Active DEA certification if state allows prescription of controlled substances by Advanced Practice Providers
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• 50% travel required

Preferred Qualifications

• One or more years of post-graduate clinical nursing experience in hospice and palliative care
• Prior experience in Family Medicine, Geriatrics, or Psychiatry
