HCA Midwest Hospice & Family Care is seeking a PRN Hospice Nurse Practitioner APRN to provide clinical assessment and management for hospice patients. In this role, you will evaluate patients for appropriateness and certification of terminal illness for Medicare/Medicaid hospice services, plan and evaluate clinical management, and provide clinical recommendations to the interdisciplinary care team.

Responsibilities

  • Review clinical materials from referring physicians to document disease process, drug regimen, and patient health assessment and prognosis at admission
  • Act as a consultant to the patient's physician and other hospice team members
  • Serve as liaison between the patient's attending physician and the interdisciplinary team, helping resolve conflicting care situations
  • Certify in conjunction with the attending physician that the patient's life expectancy does not exceed six months
  • Recertify limited life expectancy at appropriate intervals
  • Evaluate needs for immediate nursing intervention, consultation, and referral to other health team members
  • Participate with the physician in formulating policies, procedures, and protocols for managing particular health problems

Clinical Setting

HCA Midwest Hospice & Family Care provides physical, emotional, and spiritual support for patients and families throughout their hospice journey. The interdisciplinary care team includes physicians, nurses, therapists, hospice aides, social workers, music therapists, spiritual counselors, volunteers, and bereavement specialists. Patients receive hospice care at home, skilled nursing centers, independent or assisted living communities, or hospitals, with regular scheduled visits and on-call support available.

Required Qualifications

  • Master's Degree
  • Minimum three to five years of nursing experience
  • Advanced Registered Nurse Practitioner (ARNP) licensure per state guidelines
