The opportunity

Job Description

Well Care Health is seeking a PRN Home Health Registered Nurse to provide compassionate, individualized care to patients in their home environments. In this role, you will work with patients across all age groups—from neonates through geriatrics—using the nursing process to assess, plan, implement, and evaluate patient care according to established policies, procedures, and nursing standards.

Responsibilities

  • Assess and monitor patients' health conditions in their home environment
  • Develop and implement individualized care plans based on patient age, diagnosis, and clinical needs
  • Educate patients and families on health management and self-care
  • Manage a caseload of patients and coordinate care with a multidisciplinary team
  • Communicate patient information effectively in accordance with agency policies and discipline-specific guidelines
  • Maintain and adjust your schedule to enhance agency performance
  • Contribute to program effectiveness and customer satisfaction
  • Demonstrate the agency's mission, vision, and values in daily practice

Required Qualifications

  • Current RN license to practice professional nursing in North Carolina or South Carolina
  • CPR certification
  • One year of RN experience with a total of two or more years of clinical experience (supplemental experience may include LPN, CNA, military medic, EMT, or related clinical work)
  • Valid North Carolina driver's license and an operational vehicle
  • Hand/eye coordination for administering injections, using computer equipment, and manipulating patient care equipment
  • Ability to communicate and be literate in English
  • Ability to transfer and maneuver objects weighing at least 50 pounds and perform frequent pushing, moving, and lifting of patients
  • Ability to assess patient conditions, formulate care plans, select appropriate interventions, evaluate patient responses, and educate patients about their condition and recovery
  • Ability to visually assess patients and utilize sight to implement and evaluate care (changing dressings, starting IVs, regulating IVs, maintaining equipment readouts)
  • Ability to hear and auscultate lung sounds, bowel sounds, and hear alarms; effectively communicate with patients, families, physicians, and staff
  • Ability to develop positive interactions with patients, families, physicians, and staff

Education

  • Graduate of an accredited or approved school of nursing with an AD, Diploma, or BSN degree

Preferred Qualifications

  • Home health nursing experience

Schedule

  • Variable Monday through Friday, weekends and holidays as needed
  • Flexible schedule to accommodate staffing needs

Physical and Environmental Conditions

  • Requires frequent pushing, moving, and lifting of patients
  • Positioning of patients, giving baths, and ambulating patients expending significant physical effort
  • Occasionally requires reaching overhead, stair climbing, and fine motor manipulation
  • Exposure to noise, bloodborne pathogens, body fluids, infectious diseases, and needle puncture wounds
  • May be exposed to dangerous animals and traffic hazards while home visiting
  • May encounter patients and situations presenting potential threats to personal safety
  • May encounter temperature changes and weather extremes
