PRN Home Health Registered Nurse (RN)

Elara Caring is seeking a PRN Registered Nurse to provide skilled nursing care to patients in their homes in the Chicopee and surrounding communities. This part-time position offers flexibility and the opportunity to work independently while delivering meaningful one-on-one patient care. PRN scheduling is available, though dependable and consistent availability is required, whether that is 4 hours per week or 12 hours per week.

Responsibilities

• Complete admissions, assessments, routine visits, recertifications, and discharges
• Develop and update individualized plans of care
• Monitor patients' conditions and communicate significant changes to the care team
• Coordinate care with physicians, caregivers, and the interdisciplinary team
• Educate patients and families about medications, conditions, safety, and self-care
• Complete accurate and timely clinical documentation
• Help prevent avoidable hospitalizations through proactive nursing care

Required Qualifications

• Current, unrestricted Massachusetts RN license
• Previous home health nursing experience required
• Ability to work independently and manage visits efficiently
• Valid driver's license, reliable transportation, and current auto insurance
• Willingness to travel throughout Bellingham and surrounding communities
• Ability to sit, stand, bend, lift, and move intermittently and lift 50 to 100 pounds
• Availability to commit to a consistent part-time schedule

Schedule

• Part-time PRN (as-needed) scheduling with flexible hours
• Consistent availability required

Compensation

• Competitive compensation of $45 to $50 per point, based on experience
• Mileage reimbursement
