PRN Home Health Registered Nurse (RN) (58861)

Interim HealthCare is hiring PRN Hospice Registered Nurses to provide home-based care in Rochester, Minnesota and surrounding areas. In this role, you will care for patients with six months or less to live, focusing on comfort, quality of life, and supporting patients and families during end-of-life care.

Clinical Setting

You will work in patients' homes as part of an interdisciplinary hospice team that includes physicians, LPNs, CNAs, home health aides, social workers, chaplains, and volunteers.

Responsibilities

• Provide direct home-based patient care focused on comfort and quality of life
• Perform patient assessments, monitor vital signs, evaluate breathing, and assess pain levels
• Administer prescribed medications and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medications, and other supplies
• Educate patients and families about prognosis, changes in the care plan, medications, and hospice goals
• Take a holistic approach to care that addresses the patient's physical, emotional, spiritual, and family needs
• Work collaboratively with the interdisciplinary team to implement the patient's plan of care

Required Qualifications

• Graduate of an accredited nursing program
• Active RN license in Wisconsin
• Minimum of one year of nursing experience
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty
• Strong communication skills

Preferred Qualifications

• Prior experience in hospice or home healthcare
