PRN Home Health Nurse - Jeffersonville, IN

KORT Home Health is seeking a PRN (as-needed) Home Health Nurse to deliver professional nursing care to patients in their homes in the New Albany, IN and Kentucky service areas. You will work with patients receiving post-operative physical therapy, fall prevention, vestibular rehabilitation, wound care, and other home health services.

As a home health nurse, you will be responsible for conducting initial patient visits, performing comprehensive assessments, and developing plans of care under physician orders. You will use OASIS documentation to admit, transfer, re-certify, and discharge patients while maintaining compliance with Medicare and Medicaid regulations. You will coordinate and supervise LVNs and HHAs in the delivery of patient care, monitor patient progress, and communicate any changes in patient condition to physicians and clinical supervisors.

Responsibilities

• Perform initial home care patient visits and re-evaluate patient needs and progress on a regular basis
• Initiate plans of care under physician orders
• Complete admission, transfer, re-certification, resumption of care, and discharge OASIS documentation
• Observe, assess, and document patient symptoms
• Monitor patient reactions and progress
• Educate patients and caregivers on disease processes, medications, plans of care, and individualized treatment plans
• Educate patients and caregivers on in-home health care techniques
• Coordinate patient services across the care team
• Supervise LVNs and HHAs during patient care delivery
• Notify physicians and clinical personnel of changes in patient condition
• Perform skills outlined in the agency's approved policy and procedure manual
• Discharge patients from skilled nursing services when discharge criteria are met
• Participate in case conferences with clinicians to ensure care coordination
• Update clinical records according to policy and procedures
• Maintain current knowledge and skills through in-service programs, continuing education, seminars, and self-study
• Adhere to all Medicaid and Medicare regulations

Required Qualifications

• Current Indiana RN license
• Minimum one year of nursing experience as an RN
• Current automobile liability insurance
• Current unrestricted driver's license
• CPR certification (must be obtained immediately after employment)
• Ability to work positively with patients, families, and staff
• Strong verbal and written communication skills
• Flexibility with diverse client assignments

Preferred Qualifications

• One year of home health nursing experience
• OASIS E experience and proficiency

Compensation

• $50.00 per visit plus mileage reimbursement
