PRN Home Health Licensed Vocational Nurse

HealthONE Healthcare at Home is seeking a Licensed Vocational Nurse (LVN) for PRN (as-needed) home health positions. In this role, you will deliver compassionate, personalized nursing care to patients in their homes as part of an interdisciplinary care team. You will work collaboratively with registered nurses, providers, therapists, and care coordinators to develop and implement individualized plans of care for patients under physician supervision.

Responsibilities

• Provide nursing care in the home, including wound care, medication administration, and support during procedures
• Document assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
• Monitor patient status and alert the care team of any changes to ensure timely, coordinated responses
• Educate patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
• Collaborate with the interdisciplinary team to deliver high-quality, personalized care in the home setting

Required Qualifications

• Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license
• Valid driver's license
• Vocational school graduate
• One year of nursing experience
• Basic Cardiac Life Support (BLS) certification must be obtained within 30 days of employment start date

Schedule

• PRN (as-needed) positions with occasional/intermittent requirements

Compensation

• Hourly wage estimate: $0.00 - $200.00 per hour
• Actual salary determined by specific skills, years of experience, and comparison to other employees in the role
