Eventus WholeHealth is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care services in skilled nursing and senior living communities on a part-time basis. This role focuses on routine rounding, proactive chronic disease management, and coordination of care across interdisciplinary teams in a value-based care model that emphasizes preventive care and improved quality outcomes.

Responsibilities

  • Provide comprehensive primary care services within skilled nursing and senior living facilities
  • Conduct routine rounding and patient assessments
  • Manage chronic diseases proactively
  • Coordinate care across interdisciplinary teams including physicians, behavioral health specialists, care coordinators, and facility staff
  • Develop and implement integrated, person-centered care plans
  • Collaborate closely with facility partners and staff
  • Utilize AI-driven clinical tools to support decision-making and enhance care delivery

Clinical Setting

You will work in skilled nursing facilities and senior living communities, providing care to geriatric populations in a collaborative, team-based environment.

Required Qualifications

  • Current NP or PA licensure in the state of practice
  • Previous professional patient care experience
  • Exceptional patient-centered service skills, including compassionate communication, active listening, and professionalism
  • Ability to build trusting relationships with residents, families, and facility staff
  • Strong collaboration skills and ability to work effectively across interdisciplinary teams

Preferred Qualifications

  • Experience in geriatrics, family medicine, or internal medicine
