Primary Care Nurse Practitioner - Slidell, LA

Primary Care Plus, a value-based care delivery organization under UnitedHealthcare, is seeking a certified Nurse Practitioner to join our growing team in Slidell, Louisiana. You will provide comprehensive primary care to our patient population, managing both acute and chronic conditions while delivering high-quality, cost-effective, and coordinated care.

Clinical Setting

You will work in an ambulatory primary care environment, conducting patient visits both in the clinic and in patients' homes as needed. This role focuses on value-based care delivery with an emphasis on quality outcomes and patient satisfaction.

Responsibilities

• Provide primary care services including ordering and evaluating diagnostic, screening, and laboratory tests
• Manage minor acute illnesses and assess, diagnose, and manage chronic conditions for your patient population
• Conduct comprehensive wellness assessments on members
• Identify deviations from normal health status and initiate and evaluate treatment for commonly occurring health problems within your scope of practice
• Prescribe medications as delegated in protocols and procedures within your licensed prescriptive authority
• Provide individualized patient teaching and counseling
• Participate in quality improvement activities and monitoring of health outcomes
• Ensure accurate coding and documentation of HEDIS, Quality STAR measures, and Risk Adjustment diagnoses
• Participate in community events including health fairs, speaking engagements, and educational lectures
• Collaborate with the marketing team to build the practice and grow business reach within the community

Required Qualifications

• Successful completion of an advanced practice Nurse Education Program
• Current licensure through the Louisiana Board of Nurse Examiners or ability to obtain licensure prior to employment
• Nurse Practitioner Certification as Adult, Geriatric, or Family NP
• CPR certification
• Valid DEA license or ability to obtain prior to employment
• Minimum 1+ years of post-graduate experience working as a Nurse Practitioner
• Clinical knowledge necessary to make recommendations concerning clinical assessment, evaluation, and management of members
• Computer literacy and experience with Electronic Medical Records

Preferred Qualifications

• Experience in Gerontology, Internal Medicine, and/or Family Medicine
• Experience in a managed care and/or ambulatory environment
• Urgent care experience

Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including local labor markets, education, work experience, and may increase over time based on productivity and performance in the role.
