Primary Care-Nurse Practitioner

We are seeking a Nurse Practitioner to join our Primary Care team in Kingman, Arizona. In this role, you will provide comprehensive, patient-centered care to individuals across the lifespan, focusing on health promotion, disease prevention, and management of chronic conditions. You will work collaboratively with physicians and multidisciplinary team members in a supportive practice environment committed to improving the health and well-being of our community.

Responsibilities

• Provide comprehensive primary care services, including routine exams, wellness visits, and management of chronic and acute conditions
• Conduct thorough patient assessments, order and interpret diagnostic tests, and develop individualized treatment plans
• Prescribe and manage medications in accordance with evidence-based guidelines
• Provide patient education and counseling on disease prevention, lifestyle modification, and health maintenance
• Collaborate with physicians and multidisciplinary team members to ensure coordinated and effective care
• Maintain accurate and timely documentation in the electronic medical record (EMR)
• Participate in quality improvement initiatives and support population health goals

Required Qualifications

• Master's degree in Nursing (MSN) with certification as a Nurse Practitioner from an accredited program
• Current and unrestricted license to practice as a Nurse Practitioner in the state of Arizona
• National certification as a Family Nurse Practitioner (FNP)
• Current Basic Life Support (BLS) certification
• Experience required
