We are seeking a Primary Care Nurse Practitioner to join our practice in Middletown, Delaware or Elkton, Maryland. In this full-time role, you will provide comprehensive healthcare services to patients with acute and chronic conditions, emphasizing patient education and preventive care to promote overall health and wellness.

Responsibilities

  • Conduct comprehensive patient assessments, including health histories and physical examinations
  • Diagnose and manage acute and chronic illnesses, conditions, and injuries
  • Develop, implement, and evaluate individualized treatment plans
  • Prescribe medications and other treatments in accordance with state and federal regulations
  • Order and interpret diagnostic tests, such as lab work and imaging studies
  • Provide patient education and counseling on preventive care, lifestyle modifications, and treatment adherence
  • Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated patient care
  • Maintain accurate and up-to-date medical records using the practice's EMR/EHR system
  • Participate in quality improvement initiatives and professional development activities

Required Qualifications

  • Master's or Doctorate degree in Nursing from an accredited program
  • Current licensure as a Nurse Practitioner in Delaware and Maryland
  • Certification as a Primary Care Nurse Practitioner (AANP or ANCC)
  • Strong diagnostic and clinical decision-making skills
  • Excellent interpersonal, communication, and organizational skills
