Generation Clinical Partners is seeking an experienced Nurse Practitioner or Physician Assistant to provide comprehensive, high-quality patient care in skilled nursing facilities, nursing homes, and assisted living communities. You will serve as a critical member of the care team, managing acute and chronic conditions, coordinating care transitions, and leading quality improvement efforts tailored to the unique needs of each resident.
Clinical Setting
You will work exclusively in senior living facilities, providing onsite care focused on improving clinical outcomes, reducing readmissions, and enhancing operational performance. The practice is physician-led, offering mentoring and collaboration from other NP/PAs and your collaborating physician.
Responsibilities
- Manage acute and chronic conditions by developing and overseeing high-quality, cost-effective treatment plans within your scope of practice
- Order and interpret diagnostic tests and prescribe medications and other treatments as appropriate
- Provide patient and family education and counseling in self-care, health promotion, maintenance, and disease prevention
- Work closely with collaborating physicians and colleagues for consultation, collaboration, and referral
- Provide clinical leadership and education to nursing and other facility staff
- Contribute to facility performance improvement efforts
- Document all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations in the provider-friendly electronic health record
- Analyze and integrate new knowledge from conferences, workshops, professional literature, and hands-on training into clinical practice
- Maintain strict confidentiality regarding protected health information and adhere to HIPAA Privacy and Security policies
- Share in on-call rotation from home with your collaborating physician
Required Qualifications
- Current valid and unrestricted RN license in Missouri and/or Illinois
- Current valid and unrestricted NP or PA license in Missouri and/or Illinois
- Minimum 2 years of experience
- Demonstrated interpersonal skills to be approachable and understandable to patients, families, and staff
- Ability to be flexible to meet the practice schedule and ensure appropriate coverage and high-quality patient care
- Collaborative practice style and ability to work harmoniously with others
- Demonstrated skill in resolving conflicts and promoting positive outcomes for patients
- Technology proficiency with cloud-based EHR systems
Preferred Qualifications
