The opportunity

Job Description

The Center for Urologic Care is seeking a Clinical Practice Nurse 2 to deliver expert, comprehensive patient care in a medical practice setting. Reporting to the Clinical Leader or Clinical Manager, you will work under the direction of practice providers to assess patients, plan and deliver treatment, evaluate care outcomes, and mentor others in the practice nursing role. This position offers a 4-day work week schedule.

You will apply knowledge of growth and development across the life span to assess clinical data, interpret patient health information, and identify age-specific care requirements. You will engage in caring relationships with patients, families, and caregivers while collaborating with the interdisciplinary health care team and using sound clinical judgment in your nursing interventions.

Responsibilities

  • Establish priorities in organizing daily activities and maintain efficient patient flow
  • Triage patient phone calls following established guidelines to identify health problems or concerns and provide quality patient care
  • Administer prescribed medications
  • Provide expert, comprehensive education to patients, families, caregivers, groups, and communities based on identified needs
  • Document health information, interventions, and outcomes in the patient electronic medical record
  • Delegate patient care activities using the five rights of delegation (right task, person, direction, circumstance, and supervision), ensure task completion and documentation, and provide feedback to delegates and management
  • Apply principles of purposeful, organized thinking based on the nursing process to make sound clinical judgments and decisions
  • Complete practice-specific clinical skill competencies on an annual basis

Required Qualifications

  • Associate Degree or Diploma in Nursing from an accredited School of Nursing
  • Active New Hampshire Board of Nursing license
  • Current American Heart Association Basic Life Support for Healthcare Providers certification or Red Cross CPR/AED for Professional Rescuers and Health Care Providers
  • Minimum of two years of prior clinical nursing experience
  • Demonstrated ability to effectively and positively interact with a multidisciplinary team, patients, and others

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Medical practice or office experience

Physical and Work Requirements

The physical demands of this position are classified as light work. You will regularly lift, carry, or push/pull less than 10 pounds; frequently lift, carry, or push/pull up to 10 pounds; and occasionally lift, carry, or push/pull up to 20 pounds.

You may be occasionally exposed to airborne pathogens, bloodborne pathogens, and bodily fluids. The noise level in the work environment is usually moderate.
