Job Description

Horizon Home Health & Hospice is seeking a Full-Time Registered Nurse to provide comprehensive home health and hospice care to patients throughout our service area in Idaho. This position offers the independence and autonomy that home-based nursing provides, with the opportunity to make a direct impact on patients' lives and support families during critical transitions.

Clinical Setting

Horizon is one of Idaho's oldest and largest home health agencies, operating for over 20 years with branches from Weiser to Twin Falls. You will work in patients' homes, providing direct nursing care and coordinating with physicians, community health resources, and interdisciplinary teams.

Responsibilities

Patient Care

  • Complete initial comprehensive assessments of patients and families to determine home care needs, including complete history of current and previous illnesses
  • Regularly re-evaluate patient nursing needs and adjust care accordingly
  • Initiate plans of care and make necessary revisions as patient status and needs change
  • Use health assessment data to determine nursing diagnoses
  • Develop care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions, including patient and family in the planning process
  • Initiate appropriate preventive and rehabilitative nursing procedures
  • Administer medications and treatments as prescribed by the physician
  • Counsel patients and families in meeting nursing and related needs
  • Provide health care instructions to patients as appropriate per assessment and plan of care
  • Identify discharge planning needs as part of care plan development and implement prior to patient discharge
  • Serve as Case Manager when assigned by Clinical Supervisor or designee, coordinating patient care for assigned caseload
  • Adhere to the organization's documentation and care procedures and standards of personal and professional conduct

Communication

  • Prepare clinical notes and update the primary physician when necessary and at least every sixty days
  • Communicate with physicians regarding patient needs and report any changes in patient condition; obtain and receive physician orders as required
  • Communicate with community health-related persons to coordinate the care plan

Additional Duties

  • Participate in on-call duties as defined by the on-call policy
  • Ensure that arrangements for equipment and other necessary items and services are available

Required Qualifications

  • Active Registered Nurse (RN) license in the State of Idaho
  • Valid Driver's License

Preferred Qualifications

  • Home Health and Hospice experience
