Physiatry / Pain Management Provider (MD or Nurse Practitioner)

Medley Health is seeking a Physician (MD/DO) or Nurse Practitioner to provide physiatry and pain management services in skilled nursing facilities and rehabilitation centers. You will work independently while collaborating with interdisciplinary teams to help patients regain function, improve mobility, and manage pain effectively.

Clinical Setting

• Skilled nursing facilities (SNF) and rehabilitation centers
• Post-acute care environment
• On-site patient visits

Responsibilities

• Perform comprehensive physiatry and rehabilitation evaluations and follow-up visits in SNF and rehab settings
• Diagnose and manage musculoskeletal, neurologic, and functional limitations common in post-acute populations
• Develop and manage individualized treatment plans focused on functional restoration, mobility goals, and pain management strategies
• Coordinate treatment plans with physical therapy, occupational therapy, and speech therapy teams
• Monitor patient progress and adjust care plans as clinically appropriate
• Document assessments and progress notes accurately and in a timely manner
• Communicate recommendations to facility clinical teams and assist with discharge planning when needed

Required Qualifications for Physicians (MD/DO)

• MD or DO degree
• Active New York medical license
• Board Certification or Board Eligibility in Physical Medicine & Rehabilitation (PM&R) preferred, or relevant experience in pain management, musculoskeletal medicine, or rehabilitation medicine
• Experience in post-acute care, rehabilitation, pain management, or related settings preferred
• Strong clinical judgment, documentation, and collaboration skills

Required Qualifications for Nurse Practitioners

• Nurse Practitioner license (Adult-Gerontology preferred; Family Nurse Practitioner considered with relevant experience)
• Active New York NP license
• Experience in post-acute care, SNF care, rehabilitation medicine, pain management, geriatrics, orthopedics, or internal medicine preferred
• Ability to work autonomously, manage a patient panel, and collaborate with interdisciplinary teams
• Strong documentation and communication skills

Schedule

• Full-time or part-time options available
• Flexible scheduling
• No nights or weekends

Compensation

• Physicians (MD/DO): $180,000 to $220,000 annually
• Nurse Practitioners: Competitive compensation based on experience and autonomy (discussed during interview)
