Per Diem Weekend / Weeknight Telehealth Nurse Practitioner

CareMed Health Solutions is seeking Nurse Practitioners to provide telemedicine services to patients living in long-term care facilities. This is a per diem, remote position offering weekend and weeknight availability.

You will provide primary and preventative care to residents in skilled nursing facilities, manage acute changes in condition, and prescribe medications including controlled substances in accordance with state law. You will order, review, and follow up on treatments and diagnostic testing, communicate regularly with nursing home staff about changes in patient condition and care plans, and work within an interdisciplinary care team to coordinate patient care.

Required Qualifications

• Current, unrestricted NP license in Kentucky, Ohio, and/or Pennsylvania
• Board certification
• DEA licensure
• Clinical background in adult, family, or geriatric specialties
• Experience working in a long-term care setting
• Ability to work with an EMR
• Ability to manage patients with advanced illness and multiple chronic conditions
• Ability to work within an interdisciplinary care team to coordinate patient care
