Per Diem Registered Nurse (RN) HOME HEALTH

Therapeutic Home Healthcare is seeking a Registered Nurse to provide direct patient care in home settings. In this per diem role, you will work with patients managing a variety of medical conditions, administering medications, monitoring patient progress, and educating patients and families on their care plans.

You will be responsible for creating and managing individualized nursing care plans, communicating with patients, families, and the medical team to support patient-centered goals, and identifying opportunities to improve the quality of care delivered.

Responsibilities

• Provide direct patient care while maintaining nursing standards and supporting patients with their medical needs
• Create and provide patient care plans as needed or requested
• Communicate with patients, families, and members of the medical team to achieve patient-care plan goals
• Attend medical conferences and courses for ongoing professional education
• Assess patient care plans and identify areas for improvement to ensure quality of care

Required Qualifications

• Bachelor's degree in nursing
• Active state license to practice as a Registered Nurse
• Minimum of six months of recent medical experience
• Ability to stand or sit for long periods of time
• Ability to push, pull, reach, and bend frequently
• Ability to lift up to 50 pounds
