Center of Illinois Home Healthcare, Inc. is seeking an experienced Per Diem Registered Nurse to provide skilled home health nursing services to patients in their homes throughout McHenry County, Illinois. This flexible position offers the opportunity to deliver individualized patient care while working independently in a field-based setting.
Clinical Setting
Center of Illinois Home Healthcare, Inc. is a Medicare-certified, ACHC-accredited home health agency providing skilled nursing, physical therapy, occupational therapy, speech therapy, and home health aide services throughout Illinois.
Responsibilities
- Provide comprehensive skilled nursing visits in patients' homes
- Perform patient assessments and develop individualized nursing care plans
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, and Discharge visits
- Accurately complete OASIS assessments and clinical documentation within required timeframes
- Educate patients and caregivers on disease management, medications, treatments, and safety
- Coordinate patient care with physicians and the interdisciplinary care team
- Monitor patient progress and promptly report changes in condition
- Ensure compliance with Medicare, CMS, ACHC, and agency policies and procedures
- Maintain accurate, timely, and complete electronic documentation
- Participate in case conferences and other meetings as needed
Required Qualifications
- Active Illinois Registered Nurse (RN) license
- Minimum of one year of home health nursing experience
- Experience with OASIS documentation
- Knowledge of Medicare home health regulations
- Current CPR certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong assessment, communication, and time-management skills
Preferred Qualifications
- Experience using Axxess EMR
- Previous Medicare-certified home health experience
