Per Diem Nurse Practitioner or Physician Assistant - SCC - Shelton, CT

Optum Home & Community Care is seeking a Per Diem Nurse Practitioner or Physician Assistant to join the Senior Community Care (SCC) program in Shelton, Connecticut. This role provides integrated care delivery to high-risk health plan members in nursing homes, senior housing, and assisted living settings. You will conduct comprehensive clinical assessments, support members during short-term sub-acute rehabilitation, and collaborate with interdisciplinary care teams to address patients' physical, mental, and social needs.

Clinical Setting

You will work within the Senior Community Care program, an integrated care delivery model serving patients in long-term care, senior housing, and assisted living environments. The program focuses on coordinating clinical care for complex, high-risk members.

Responsibilities

• Conduct comprehensive assessments including past medical history review, medication reconciliation, vital signs, and physical examination
• Perform evidence-based screenings and point-of-care testing as appropriate
• Support members during short-term sub-acute rehabilitation episodes
• Ensure accurate and complete documentation, including ICD-10 coding
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Schedule

• Monday through Friday within 8 a.m. to 5 p.m. timeframe (hours flexible)
• No on-call, weekends, or holidays required
• Per diem employment type

Required Qualifications

• For Nurse Practitioners: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For Physician Assistants: Current NCCPA certification and state licensure
• Active, unrestricted state licensure in Connecticut (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver's license with access to reliable transportation
• Current, active DEA licensure or prescriptive authority (or ability to obtain post-hire per state regulations)
• Ability to gain a collaborative practice agreement if applicable in your state
• Ability to navigate varied environments and position oneself as needed to perform job duties

Preferred Qualifications

• One or more years of clinical experience as an Advanced Practice Clinician, preferably in a long-term care setting
• Experience in geriatric medicine, long-term care, senior living, or home care settings
• Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs
• Understanding of geriatrics and chronic illness management
• Understanding of advanced illness and end-of-life discussions
• Proficiency with electronic medical records and technology

Compensation and Benefits

• Hourly compensation range: $53.00 to $79.00 per hour
• $5,000 sign-on bonus for external candidates
• Total cash compensation includes base pay and bonus, based on local labor markets, education, work experience, and may increase based on productivity and performance
