Per Diem Licensed Practical Nurse (LPN)

Rock River Nursing & Rehab is seeking a Per Diem Licensed Practical Nurse (LPN) to deliver high-quality patient care in our long-term care facility. As a per diem LPN, you will have the flexibility to choose your shifts and work schedule while making a meaningful impact on the lives of our residents.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring, under the supervision of a registered nurse
• Assist with daily living activities, such as bathing, dressing, and toileting, to promote comfort and dignity
• Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident
• Maintain accurate and detailed records of patient care and treatment
• Participate in quality improvement initiatives and contribute to the development of a positive and respectful work environment

Required Qualifications

• Current LPN licensure in the state of Wisconsin
• Strong clinical skills and ability to work effectively in a fast-paced environment
• Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare team members
• Ability to work flexible hours, including evenings, weekends, and holidays

Schedule

• Per diem position with flexible scheduling
• Availability for evenings, weekends, and holidays required
