Pediatric Home Health Licensed Practical Nurse or Registered Nurse

Golden Care of Northeast PA is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide pediatric home health nursing care. In this role, you will deliver professional nursing care to assigned pediatric patients in their homes, working under the supervision of a registered nurse or physician. You will assess patients' physical and emotional status, implement care plans, and coordinate with families and clinical management to ensure quality care.

Responsibilities

• Deliver professional nursing care to assigned pediatric patients
• Assess patients' physical and emotional status, competency, home environment, safety factors, and family support
• Record accurate patient medical history and maintain clinical records and reports
• Take and record vital signs including blood pressure, temperature, and heart rate
• Monitor patients' condition including fluid intake and output; compose and update patient charts
• Observe patients under treatment to identify progress and medication side effects
• Perform nursing procedures and techniques as ordered by the physician, including administering medications and intravenous solutions
• Administer injections, prescribed medications, enemas, and other basic medical care tasks
• Clean and dress wounds as needed
• Collect samples for lab testing as assigned
• Assist patients with activities of daily living such as eating and bathing
• Provide emotional and psychological support when needed
• Communicate with patients' families and friends to provide advice, comfort, and discharge instructions
• Provide information on patient rights, reimbursement procedures, and family rights
• Supervise Home Health Aides and ensure quality care is provided
• Oversee patient care including direction, coordination, and evaluation of care and staff
• Participate in on-call rotation as required
• Perform other duties as assigned by management

Required Qualifications

• Active LPN or RN license in good standing in Pennsylvania
• Graduate of an accredited school of nursing
• Proven experience as an LPN, RN, or similar role
• Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
• Familiarity with hospital health, safety, and sanitation standards and procedures
• Understanding of confidentiality obligations and nursing best practices
• Excellent knowledge of medical and hospital terminology
• Solid knowledge of MS Office and data entry
• Strong nursing, observation, communication, and supervision skills
• Knowledge of home health care and government regulations
• Good physical and mental health
• Caring attitude, empathy, tact, and patience
• Ability to solve practical problems and interpret various instructions
• Perfect physical condition and stamina
• Excellent communication and interpersonal skills
• Ability to handle stress

Preferred Qualifications

• At least one year of professional nursing experience

Physical Demands

This position requires moderate physical exertion including driving to patient homes, assisting in transfer activities, and providing support in ambulation therapy. You will be regularly required to use hands, talk, and hear; frequently required to stand, walk, reach, stoop, kneel, crouch, or crawl. You must occasionally lift and/or move over 100 pounds. Vision requirements include close vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
