Pediatric Cardiothoracic Nurse Practitioner - (BCH-Oakland/San Francisco)

UCSF Benioff Children's Hospital is seeking a Pediatric Cardiothoracic Nurse Practitioner to deliver comprehensive healthcare to pediatric cardiothoracic surgery patients. You will work under the clinical supervision of the Chief of Pediatric Cardiothoracic Surgery and administrative supervision of the Director of Pediatric Critical Care, collaborating with an interdisciplinary team across inpatient and outpatient settings.

Clinical Setting

You will primarily work at UCSF Benioff Children's Hospital – San Francisco campus with potential cross-training at the Oakland campus or other facilities where pediatric cardiothoracic surgeons operate. Care is provided in the Pediatric Cardiac Transitional Care Unit, inpatient areas, outpatient clinics, via telehealth, and potentially in the operating room depending on departmental need, experience, and interest.

Responsibilities

Inpatient Direct Patient Care (70% of time):

• Independently or collaboratively evaluate and manage patients, including taking histories, performing physical examinations, and responding to changes in patient status
• Order or furnish medications, diagnostic tests, treatments, therapies, consultations, referrals, durable medical equipment, and home health services
• Perform procedures granted through privileges or standardized procedures
• Identify patients with acute decompensation and respond to hospital plans of care
• Initiate consultations as indicated
• Document independent assessment findings and recommend appropriate treatments
• Participate in service rounds and patient evaluation and treatment planning
• Review lab tests, x-rays, and other diagnostic tests, and initiate appropriate team plans
• Evaluate complex cases and establish individualized plans of care for disposition to lower levels of care
• Interpret and act upon completed tests, procedures, and radiographic studies
• Complete clinically relevant paperwork and electronic health record documentation
• Effectively communicate findings, treatment options, and care plans with patients and families
• May first assist in the operating room if privileged to do so after completing required certification and proctoring
• Monitor and evaluate patients for discharge, setting parameters and guidelines for continuum of care
• Discharge patients including prescribing or furnishing medications, ordering durable medical equipment, recommending or referring for ongoing follow-up care, providing discharge teaching, and completing discharge summaries

Inpatient Care Coordination (25% of time):

• Coordinate care throughout the continuum including hospital admission, care coordination with staff and consulting services, and hospital discharge planning
• Coordinate episodic care and determine timing of routine clinic follow-up
• Participate in huddles with clinic and hospital staff and anticipate patients requiring close management or intervention
• Consult with nursing staff on plan and implementation of individualized nursing care plans
• Independently provide case summaries for disability, insurance agencies, workers compensation, work releases, and medical supply agencies
• Formulate and implement protocols to improve quality and efficiency and control waste in the clinical specialty

Outpatient Independent Encounters (5% of time):

• Evaluate and manage patients in independent encounters, including telehealth
• Provide care to new, follow-up, acute, routine, and specialty patients
• Take histories, perform physical examinations, interpret diagnostic tests, formulate evidence-based assessments and plans, and respond to changes in patient status
• Order medications, diagnostic tests, treatments, therapies, consultations, referrals, and durable medical equipment or home health services
• Review or collaborate with attending physician as needed
• Perform procedures individually granted through standardized procedures or privileges
• Identify high-risk groups and complex cases for review with attending physician
• Initiate consultations as appropriate
• Evaluate complex cases and establish individualized plans of care for rehabilitation
• Facilitate occupational and physical therapy and physical medicine and rehabilitation consultations
• Interpret and act upon ordered tests, procedures, and radiographic studies in a timely fashion
• Complete clinically relevant paperwork and electronic health record documentation

Required Qualifications

• Graduation from a California Board of Registered Nursing approved or verified school of nursing with a master's degree in nursing or a Doctor of Nursing Practice (DNP)
• Minimum of six months of NP experience in Pediatric Critical Care or Pediatric Cardiology
• Demonstration of advanced interpersonal skills while working with families of acutely and critically ill children
• Excellent organization and communication skills

• Ability to critically think and problem solve

Preferred Qualifications

• One year of working experience as an NP in pediatric cardiology
• RNFA certificate

Licenses and Certifications

• California Registered Nurse (RN) License without restriction
• California Certified Nurse Practitioner (NP) License without restriction
• California Nurse Practitioner Furnishing Number including Schedule II furnishing privileges
• NP national board certification from an accepted certifying agency aligned with the patient population and clinical role (Pediatric Inpatient – CPNP-AC)
• California Driver License (if required for position)
• Drug Enforcement Administration (DEA) Registration with Schedules 2, 2N, 3, 3N, 4, and 5 registered to a California address
• American Heart Association (AHA) Basic Life Support (BLS) certification
• American Heart Association (AHA) Pediatric Advanced Life Support (PALS) certification

Credentialing Requirement

UCSF Medical Center requires all Advanced Practice Providers to be credentialed through its Office of Medical Affairs and Governance (OMAG) to practice and function in their clinical role. This applies to both adult and pediatric APPs in inpatient and outpatient clinical settings. Credentialing takes between 60-90 days and must be completed prior to the first working day.
