The opportunity

Job Description

CareMed Health Solutions is seeking a Part Time Wound Care Nurse Practitioner to provide specialized wound services to patients residing in skilled nursing facilities in the long-term care setting. You will work as part of an interdisciplinary care team, delivering quality wound management to the elderly population while collaborating with facility nursing staff and primary care physicians.

Clinical Setting

  • Long-term care facilities and skilled nursing facilities
  • Patient population: elderly residents requiring wound care services

Responsibilities

  • Provide wound care services for long-term care patients
  • Prescribe appropriate medications, including controlled substances in accordance with Ohio law
  • Order, review, and follow up with treatments and diagnostic testing
  • Communicate regularly with nursing home staff regarding changes in patient condition and care plan
  • Collaborate with primary care physicians on patient care coordination
  • Diagnose and treat diabetic, arterial, venous, and mixed etiology wounds, as well as Kennedy Ulcers
  • Stage pressure injuries according to NPIAP standards
  • Manage advanced wound care products and devices

Required Qualifications

  • Current, unrestricted Nurse Practitioner license in Ohio
  • Board Certification
  • Clinical background in adult, family, or geriatric specialties
  • Clinical background in wound care, including chronic, debilitating, stalled, or infected wounds
  • Comfort with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers
  • Comfort staging pressure injuries according to NPIAP standards
  • Familiarity with wound care best practices and advanced wound care products (collagens, alginates, foams, silvers, etc.)
  • Experience with wound vacs and other DME such as pressure relieving/reducing devices
  • Experience working in a long-term care setting
  • Ability to work with an EMR
  • Ability to work within an interdisciplinary care team to coordinate patient care

  • DEA licensure
  • Wound care certification or openness to obtaining certification
