Greater Good Health is seeking a Part Time Telehealth Nurse Practitioner to provide high-quality care to older adults in the Southern Oregon community. This role focuses on managing high-risk patients through telemedicine to prevent unnecessary acute care admissions, optimize medical condition management, and address clinical and psychosocial factors contributing to healthcare utilization. You will work 4-6 days per month on an 8am-5pm PST schedule, entirely remote.
Clinical Setting
This is a telemedicine-based position serving senior Medicare patients in access-starved communities. You will work collaboratively within an interdisciplinary team model that includes social workers, case managers, and other clinicians to ensure holistic patient care.
Responsibilities
- Assess patients' healthcare needs and provide clinical care and consultation via telemedicine
- Perform annual wellness exams for senior Medicare patients via telemedicine, including obtaining and recording medical history, symptoms, and medication reconciliation
- Perform HCC coding, verification, and risk adjustment
- Coordinate continuity of care, prevention of complications, and clinical treatment and management under the direction of patients' primary healthcare providers and in collaboration with referral sources
- Perform various tests and screenings via telemedicine
- Counsel patients on discharge plans and provide education
- Execute discharge plans and use critical thinking skills to build effective care plans during critical transition periods after hospital discharge
- Support patients and families with compassionate care
- Contribute to interdisciplinary team meetings and collaborate with other healthcare professionals to ensure holistic care plan implementation
- Complete charts within 72 hours of patient visits using EMR and/or portals
- Cover for other Nurse Practitioners and care team members as needed
- Maintain patient confidentiality, safeguard medical records, and comply with all HIPAA regulations
Required Qualifications
- Current Oregon Nurse Practitioner license (required)
- Nurse Practitioner certification from ANCC or AANP
- Current DEA licensure and prescriptive authority
- Active CPR/BLS certification
- Minimum 2 years of clinical experience as an NP in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care
- Working knowledge of chronic care management and various medication options

Schedule
- Part time: 4-6 days per month
- Hours: 8am-5pm PST
- Entirely remote (Oregon residents only; employer hires in select states)
