Part-Time Nurse Practitioner (VSP)

Join CenterWell Senior Primary Care, part of Humana's Primary Care Organization, as a Part-Time Nurse Practitioner providing virtual primary care services to seniors in Upstate South Carolina. Clinical Setting

You will support 340+ centers across 15 states under the CenterWell and Conviva brands, delivering value-based primary care to adult and geriatric patients through virtual services. The care model emphasizes preventive care, wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment, with integrated teams including physicians, nurses, and behavioral health specialists.

Responsibilities

• Provide virtual primary care services for patients supporting Upstate South Carolina centers
• Report to the Associate Medical Director
• Maintain accurate documentation and medical records
• Coordinate referrals and collaborate with specialists
• Partner with interdisciplinary teams for holistic, whole-person care

Required Qualifications

• Graduate of an accredited Nurse Practitioner program (MSN or DNP)
• Current national certification (AANP or ANCC)
• Active, unrestricted NP license in South Carolina
• DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire)

Preferred Qualifications

• 2 or more years of Nurse Practitioner experience
• Experience in primary care or Medicare Advantage settings with geriatric patients
• Experience with Athena EMR
• Familiarity with HEDIS quality indicators and Medicare guidelines
