Aveanna is seeking a Licensed Practical Nurse to provide skilled nursing care to patients in their homes across Decatur and Athens in Morgan and Limestone Counties. This is a part-time PRN position where you will deliver quality patient care in accordance with physician orders and individualized care plans while working under the direction of a Registered Nurse teammate.
Clinical Setting
You will provide care in patients' homes, serving patients who require skilled nursing services covered by Medicare and Medicaid.
Responsibilities
- Deliver quality patient care in compliance with physician orders under direction of your Registered Nurse teammate
- Perform hands-on client care through documentation and timely communication with the client's physician and other caregivers
- Recognize changes in patient needs and responses requiring intervention and implement care to prevent or reduce risk
- Educate clients and their family members based on the client's specific needs
- Provide a variety of skilled nursing services including phone triage, patient education, observation and assessment, wound care, infusions, catheter care, and PICC line dressing changes
Schedule
- PRN visits as needed
- Territory covers Decatur and Athens in Morgan and Limestone Counties
Required Qualifications
- Active LPN license in the state of application
- Valid driver's license in the state of application
- Valid CPR certification
Preferred Qualifications
- Medicare Skilled Nursing experience
- Basic understanding of OASIS
- One year of LPN experience in a home health or healthcare setting
- Home Care Home Base experience
