Part Time Licensed Practical Nurse Home Health PRN

Aveanna is seeking a Licensed Practical Nurse to provide skilled nursing care to patients in their homes across Decatur and Athens in Morgan and Limestone Counties. This is a part-time PRN position where you will deliver quality patient care in accordance with physician orders and individualized care plans while working under the direction of a Registered Nurse teammate.

Clinical Setting

You will provide care in patients' homes, serving patients who require skilled nursing services covered by Medicare and Medicaid.

Responsibilities

• Deliver quality patient care in compliance with physician orders under direction of your Registered Nurse teammate
• Perform hands-on client care through documentation and timely communication with the client's physician and other caregivers
• Recognize changes in patient needs and responses requiring intervention and implement care to prevent or reduce risk
• Educate clients and their family members based on the client's specific needs
• Provide a variety of skilled nursing services including phone triage, patient education, observation and assessment, wound care, infusions, catheter care, and PICC line dressing changes

Schedule

• PRN visits as needed
• Territory covers Decatur and Athens in Morgan and Limestone Counties

Required Qualifications

• Active LPN license in the state of application
• Valid driver's license in the state of application
• Valid CPR certification

Preferred Qualifications

• Medicare Skilled Nursing experience
• Basic understanding of OASIS
• One year of LPN experience in a home health or healthcare setting
• Home Care Home Base experience
