The opportunity

Job Description

HospiceCare is seeking a Palliative Care Nurse Practitioner to provide advanced clinical care to patients receiving palliative and hospice services. In this role, you will assess, treat, and manage patients utilizing advanced assessment and evaluation techniques, provide counseling and patient education, and deliver coverage for hospital palliative care services as directed by the Palliative Care Medical Director and Director of Palliative Care.

  • 32 hours per week
  • Exempt status

Location

  • Charleston, WV

Responsibilities

  • Assess and treat palliative care patients using advanced clinical techniques
  • Solve problems and provide counseling to patients and families
  • Educate patients and families about their care
  • Provide coverage for hospital palliative care services at the discretion of leadership
  • Perform related duties as assigned

Required Qualifications

  • Graduate of an approved nursing program
  • Successful completion of an accredited university-affiliated Nurse Practitioner program
  • Current unrestricted West Virginia RN license with advanced practice designation
  • Strong organizational, problem-solving, and people skills

Physical Requirements

  • Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance
  • Frequent standing and walking for extended periods (8-12 hours per shift)
  • Frequent bending, stooping, kneeling, crouching, and reaching
  • Ability to push and pull equipment such as wheelchairs, stretchers, and medication carts
  • Visual acuity to read medication labels, physician orders, and monitors
  • Auditory ability to assess patient needs and respond to alarms or calls for assistance

Work Environment

  • Exposure to infectious diseases, bloodborne pathogens, and bodily fluids
  • Frequent interaction with patients, families, and interdisciplinary team members
  • Fast-paced clinical settings with potential for high stress
  • Required use of personal protective equipment (PPE)
