Palliative Care Nurse Practitioner

We are seeking a Nurse Practitioner to provide comprehensive palliative and advanced disease management services to patients with serious or life-limiting illness. In this role, you will work collaboratively with patients' primary care providers and an interdisciplinary team to deliver compassionate, patient-centered care in community settings and through telehealth. You will assess patient conditions, manage complex medications and treatments, educate patients and caregivers about disease progression and goals of care, and help facilitate transitions between healthcare settings.

Responsibilities

• Perform comprehensive patient assessments, including history and physical examinations
• Order and interpret laboratory and diagnostic tests
• Diagnose health conditions and document findings in medical records
• Develop individualized plans of care for patients with chronic and complex conditions
• Prescribe medications and treatments in collaboration with the supervising physician
• Obtain consultations and coordinate referrals with community care services
• Educate patients and caregivers regarding medications, side effects, treatment options, illness progression, diet, nutrition, and medical adherence
• Assist patients and caregivers in understanding and completing advance directives based on patient goals and wishes
• Facilitate clarification of patient and caregiver goals of care
• Implement interventions to reduce recurring hospitalizations and monitor their effectiveness
• Facilitate patient transitions between healthcare settings
• Collaborate with the interdisciplinary team to ensure appropriate symptom management
• Review and develop treatment protocols in collaboration with the supervising physician
• Consult with the supervising physician when the plan of care falls outside standardized practice and protocols
• Communicate and collaborate with the interdisciplinary team to create, review, and revise care plans
• Maintain accurate and timely clinical documentation, including assessment notes, medical orders, and collaboration notes
• Participate in interdisciplinary team meetings
• Serve as a nursing resource for consultation and education to team members and community healthcare practitioners
• Build and maintain relationships with staff and community referral sources
• Maintain knowledge of and compliance with Medicare/Medicaid, state, and federal regulations
• Ensure compliance with Medicare conditions of participation and state healthcare regulations
• Comply with all HIPAA requirements
• Participate in quality monitoring and quality improvement initiatives
• Pursue continuing education and professional development

Clinical Setting

• Community-based care and telehealth
• Work under the supervision of a collaborating physician and direction of a medical director

Required Qualifications

• Graduation from an accredited School of Nursing
• Completion of an accredited Nurse Practitioner Program conforming to state Board educational standards
• Current nursing licensure in your state
• Current CPR certification
• Master's degree
• Minimum one year of Nurse Practitioner experience
• Minimum one year of hospice or palliative care experience
• Accreditation from an approved certifying body for advanced practice nursing as required by your state
• Ability to perform duties autonomously and meet workload expectations
• Ability to provide leadership for the care team
• Excellent observation, verbal, and written communication skills

• Ability to walk or stand for prolonged periods
• Ability to lift, position, and transfer patients
• Ability to lift supplies and equipment
• Ability to reach, stoop, bend, kneel, and crouch as needed
• Adequate visual acuity and hearing to perform required nursing skills
• Valid driver's license
• Personally owned, insured automobile in good working order
• Ability to travel between care sites

Preferred Qualifications

• Specialist certification such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics

Licenses and Certifications

• Current state nursing license
• CPR certification
• Accreditation from an approved certifying body for advanced practice nursing as required by state

Travel Requirements

• Travel between care sites is mandatory
• Must maintain a valid driver's license and personally owned, insured automobile in good working order
