Outpatient Palliative Care Nurse Practitioner or Physician Assistant

NJ Palliative Care Solutions is hiring a Nurse Practitioner or Physician Assistant to provide outpatient and skilled nursing facility-based palliative care across Ocean County, New Jersey. You will deliver high-touch, patient-centered palliative care to patients facing serious illness in SNF, home, and community settings, with structured mentorship, physician/APP backup, and comprehensive team support.

Clinical Setting

You will work across multiple settings within Ocean County, including Toms River, Brick, Jackson, Manchester, and Whiting. Your practice will span skilled nursing facilities, outpatient consultations, and community-based care. You will manage both short-stay subacute and rehabilitation patients with recent hospitalizations, high symptom burden, or complex goals-of-care needs, as well as ongoing management of long-term care residents.

Responsibilities

• Perform best practice, holistic, patient-centered, evidence-based interdisciplinary palliative medicine at the top of your license
• Provide palliative care consultations and follow-up visits across skilled nursing settings
• Deliver palliative assessments and follow-up visits in SNFs and community settings
• Manage complex symptoms for patients with advanced illness and chronic disease
• Lead goals-of-care and advance care planning conversations with patients and families
• Coordinate with partner home health and hospice agencies as appropriate
• Partner with the interdisciplinary team, facility staff, primary care providers, and hospice
• Support transition planning across the SNF stay, from admission after hospitalization through discharge home, return to long-term care, rehospitalization decisions, hospice transition, or other next-site-of-care planning
• Complete timely documentation aligned with clinical, billing, and quality standards
• Travel locally within Ocean County with mileage reimbursement and clinical/EMR support provided

Required Qualifications

• Active New Jersey Nurse Practitioner or Physician Assistant license
• National board certification
• DEA and CDS registration, or ability to obtain by start date
• Two or more years of clinical experience

Preferred Qualifications

• Palliative care experience
• Palliative care certification such as ACHPN or CHPN
• Clinical background from oncology, critical care, hospital medicine, geriatrics, SNF/LTC, primary care, or home-based care
• Strong communication skills, clinical judgment, and comfort working on interdisciplinary teams

Schedule

Primarily Monday through Friday with flexibility to swap workdays. Full-time is strongly preferred. For exceptional candidates, part-time or per-diem schedules may be considered.

Compensation and Benefits

• Base salary: $130,000 to $160,000 per year
