Pathways Healthcare, a physician- and nurse practitioner-led home health and hospice organization, is seeking an On Call Hospice Registered Nurse for weekday and night shift coverage in the Wilkes Barre area. This part-time position offers the opportunity to provide direct in-home hospice care to patients while working flexible on-call hours.

Clinical Setting

You will provide care to hospice patients in their homes, working as part of an interdisciplinary team led by physicians and nurse practitioners. The organization specializes in comprehensive, patient-centered care with a focus on helping patients maintain dignity and quality of life.

Schedule

  • Monday to Friday, 5:00 pm to 8:30 am
  • On-call availability for weekend calls from 8:30 am to 8:00 pm
  • Part-time position

Responsibilities

  • Initiate appropriate preventive and rehabilitative nursing procedures
  • Administer medications and treatments as prescribed by the physician
  • Initiate the plan of care and make necessary revisions as patient status and needs change
  • Respond promptly to incoming calls during after-hours
  • Assess patient needs through triaging and provide in-home care when needed
  • Collaborate with physicians, nurse practitioners, and the interdisciplinary team to develop and maintain strong plans of care
  • Respond to inquiries and concerns from patients, caregivers, and medical team members
  • Consult with patients and caregivers before, during, and after treatment

Required Qualifications

  • Degree in Nursing from an accredited institution
  • Active RN license
  • Current BLS CPR certification
  • Valid driver's license and reliable, insured vehicle for in-home visits
  • Proficiency with electronic medical records
  • Working knowledge of health care laws and regulations
  • Excellent bedside manner and communication skills

Preferred Qualifications

  • Two or more years of nursing experience
  • Experience in geriatrics
  • New graduates are encouraged to apply; the organization offers a comprehensive training program
