The opportunity

Job Description

This position supports an RN Case Manager in a home health office setting, providing administrative and clinical coordination tasks within the Licensed Practical Nurse scope of practice. You will assist with patient care management, clinical documentation, and care coordination to ensure accurate and timely information flow across the nursing team.

Clinical Setting

  • Primary work location: Office-based with occasional patient home visits
  • Setting: Home health agency

Responsibilities

  • Assist the Case Manager with phone communications, computer documentation notes, and preparation of information for weekend staff
  • Perform daily follow-up with hospitals for status updates on admitted patients
  • Contact field staff with all information affecting patients' Plans of Care
  • Print and distribute field staff daily and weekly schedules and pertinent information
  • Conduct daily check-ins with field staff regarding missed visits and adjust schedules as needed
  • Fax physician-required information including missed visit reports, fall sheets, and abnormal findings
  • Prepare wound care notes and supply copies to field staff
  • Complete required forms and processes including Infection Control Sheets for patients on antibiotics and Post Fall Sheets, forwarding to appropriate departments
  • Receive and process reports submitted by field staff
  • Generate Medication Administration Records (MARS) for patient re-certifications and supply copies to field RN
  • Perform 14-day phone monitoring assessments for high-risk re-hospitalization patients
  • Record medication list changes and verify with physicians as needed
  • Prepare orders and communication notes and update field folders accordingly
  • Ensure lab work is completed daily and results are faxed to physicians with follow-up on new orders
  • Perform other duties as assigned by management

Required Qualifications

  • Valid Louisiana Licensed Practical Nurse (LPN) license without restrictions
  • One year of healthcare or home health-related experience
  • Associate's degree or higher
  • Demonstrated ability to use provided software and computer systems
