Oak Street Health is seeking new graduate Nurse Practitioners and Physician Assistants for a 12-month paid fellowship in Adult Primary Care. This program, offered in formal collaboration with the University of Michigan School of Nursing, is designed to launch your career in a value-based care model that prioritizes quality of care over volume of services. You will work with older adults on Medicare in a community-based primary care setting.
The fellowship provides comprehensive training, support, and didactic content to build confidence and competence as a clinician. You will spend time building your own patient panel, learning from multiple preceptors, and engaging in structured education that builds on your graduate training rather than repeating it. The program emphasizes small patient panels and true interprofessional collaboration to ensure optimal health outcomes.
Fellows commit to working at Oak Street Health for at least one year following program completion, with opportunities to continue your career with the organization. Two concurrent cohorts begin each year in September and March.
