Oak Street Health is seeking new graduate Nurse Practitioners to join its 12-month paid fellowship program in Adult Primary Care. This formal collaboration with the University of Michigan School of Nursing is designed to launch your NP career in a value-based care model that prioritizes quality of care over volume of services.
You will work with older adults on Medicare in a primary care setting across Oak Street Health's network of over 160 locations. The fellowship provides comprehensive training, support, and didactic content to build your clinical confidence and competence. You will spend time building your own patient panel, learning from multiple preceptors, and engaging in structured education that builds on your graduate training rather than repeating it.
The program operates on two concurrent cohorts starting in September and March each year. Fellows commit to at least one additional year of employment with Oak Street Health following the 12-month fellowship, with opportunities to continue your career with the organization.
Oak Street Health operates a value-based primary care model centered in patient communities, focused on quality outcomes for older adults on Medicare. You will work as part of a true interprofessional care team with dedicated time for collaboration to ensure optimal patient health outcomes. Patient panels are kept small to support quality care delivery.
