Nurse Practitioner/Physician Assistant- Leesburg

Community Health Centers is seeking a Nurse Practitioner to provide comprehensive primary care in an ambulatory setting. You will deliver preventive and health maintenance services, manage acute and chronic conditions, and perform clinical evaluation, diagnosis, and treatment for patients across our modernized health centers.

Clinical Setting

You will work as a collaborative member of the healthcare team in outpatient primary care environments. The role involves working with diverse patient populations and may require rotation to other Community Health Centers locations as needed.

Responsibilities

• Provide comprehensive outpatient primary care services including preventive care, health maintenance, acute and chronic disease management, clinical evaluation, diagnosis, and treatment
• Adhere to current disease management protocols and clinical pathways to improve clinical performance
• Complete required documentation and assigned tasks on time, including EHR processes, telephone encounters, chart management, and payroll documentation
• Facilitate necessary referrals, including social services, in coordination with referral services representatives and care team nurses
• Review incoming reports (labs, x-rays, EKGs) and follow up per guidelines
• Maintain current knowledge of coding and billing requirements and provide accurate coding information in the EHR
• Perform surgical procedures using standard sterile techniques and CDC guidelines within scope of practice
• Participate in team member outreach and off-site health care programs
• Precept medical students as assigned
• Participate in peer review and quality assurance activities
• Attend mandatory Community Health Centers meetings including center-specific quarterly meetings and corporate meetings
• Rotate to other centers and provide off-hours and Saturday clinic coverage as assigned
• Comply with Community Health Centers policies, procedures, and state and federal regulations
• Understand and comply with the Exposure Control Plan and utilize universal precautions
• Participate in collaboratives and health care programs approved by the Executive Team
• Act as a resource for medical support staff and provide support to clinical staff and center leadership
• Assist with recruitment of physicians and clinical staff
• Make recommendations regarding clinical supplies, instruments, equipment, and maintenance
• Demonstrate ability to work cooperatively with clinician team members
• Maintain professional appearance at all times

Required Qualifications

• Master's degree or Post-Master's degree certification (required for applicants who graduated on or after October 1, 1998; applicants who graduated prior to October 1, 1998 are exempt from this requirement)
• Current certificate issued by ANCC and/or AANP or eligible for certification
• Active Florida license
• BLS certification
• High degree of responsibility with excellent interpersonal skills, organizational ability, and problem-solving skills
• Strong written and oral communication skills
• High degree of accuracy, attention to detail, and manual dexterity
• Ability to quickly identify problems and their most effective treatments
• Skilled in addressing patient care needs in a compassionate and empathetic manner
• Skilled in working in a collaborative, team-based practice
• Ability to manage patient expectations and complaints professionally
• Computer proficiency including Microsoft Office, Internet, electronic health records, and online resources
• Logical, methodical approach to work with meticulous record-keeping
• Commitment and ability to keep up to date with scientific and technological developments and literature
• Ability to work flexible hours as assigned
• Ability to work at other Community Health Centers locations as assigned

Preferred Qualifications

• Three to five years of clinical experience
• DEA license
• Knowledge of accreditation standards (AAAHC, HRSA)
• Bilingual capability

Background Screening

This position requires background screening through the Florida Care Provider Background Screening Clearinghouse.
