The opportunity

Job Description

Vanderbilt Health's Adult Congenital Heart Disease Program is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver specialized care to patients with adult congenital heart disease. This is an outpatient position within one of the region's most experienced programs, recognized as a comprehensive care center by the Adult Congenital Heart Association.

Clinical Setting

You will work in an ambulatory specialty setting focused on adult congenital heart disease. The program provides comprehensive care to help patients live healthy, active lives and is part of Vanderbilt Health's eight-hospital system with over 180 clinics across the Mid-South region.

Responsibilities

  • Obtain and document comprehensive health histories and perform system-focused or symptom-specific physical examinations
  • Develop assessments and plans of care in collaboration with the healthcare team
  • Manage primary, acute, and complex care for patients, identifying expected outcomes for diagnoses
  • Order, perform, interpret, and collect diagnostic data using appropriate assessment techniques and diagnostic procedures
  • Form differential diagnoses and treat acute and chronic conditions
  • Prescribe therapeutic interventions including pharmacologic, non-pharmacologic, and surgical treatments to achieve expected outcomes
  • Utilize evidence-based, approved practice protocols in planning and implementing care
  • Initiate appropriate referrals and consultations
  • Provide specialty-specific consultation services within your scope of practice
  • Facilitate patient transitions between and within healthcare settings
  • Clearly document findings, assessments, and plans of care in the electronic health record
  • Educate patients, families, and healthcare team members on clinical topics

Required Qualifications

  • Active Tennessee license as a Nurse Practitioner or Physician Assistant
  • Active Tennessee RN license (for NP candidates)
  • Master's degree
  • Adult-Gerontology Acute Care Nurse Practitioner (AGACNP), Acute Care Nurse Practitioner (ACNP), or Physician Assistant (PA) certification
  • Relevant work experience
