We are seeking a Nurse Practitioner to provide primary care to residents in assisted living communities in Queens, New York. This is a part-time, on-site position with no nights or weekends, offering the opportunity to deliver compassionate, patient-centered care to seniors while maintaining work-life balance.
Clinical Setting
You will work in assisted living communities serving senior residents, conducting full health assessments, managing chronic and acute conditions, and collaborating with interdisciplinary care teams.
Responsibilities
- Conduct comprehensive health assessments, physical exams, and medication reviews
- Create personalized care plans in collaboration with the interdisciplinary team
- Manage chronic and acute conditions
- Order and interpret laboratory and diagnostic tests
- Prescribe medications within your NP scope of practice
- Educate residents and families on health maintenance and independence
- Maintain clear, up-to-date clinical records in eClinicalWorks
- Participate in quality improvement initiatives
Required Qualifications
- Master's degree from an accredited Nurse Practitioner program
- Active New York State NP license
- DEA registration
- Eligible for Medicare/Medicaid billing
- Minimum 3 years of clinical experience
Preferred Qualifications
- Geriatric or long-term care experience
Schedule
- Part-time with flexible hours
- On-site only (not remote)
- No weekends
- No nights
Compensation
- $60,000 to $120,000 per year, depending on experience and hours worked
- W2 and 1099 compensation models available
