The opportunity

Job Description

This position offers an opportunity to provide comprehensive nephrology care to patients with end-stage kidney disease (ESKD) and chronic kidney disease (CKD) in a physician practice setting in San Antonio. You will work collaboratively with nephrologists, dialysis center teams, and other healthcare providers to deliver holistic patient care and disease management.

Responsibilities

  • Conduct ESKD rounding, including limited and comprehensive patient visits under the guidance of practice nephrologists
  • Manage ESKD patients holistically, including patient activation, depression screening, hospitalization risk assessment, transition of care coordination, and co-morbid medical issue oversight
  • Provide ESKD and CKD patient education
  • Perform comprehensive disease management annual visits for ESKD patients
  • Collaborate with nephrologists, dialysis center interdisciplinary teams, Kidneylink IDT, hospital discharge care coordination teams, primary care providers, and subspecialists as clinically indicated
  • Perform physical examinations to evaluate health and diagnose illnesses
  • Order and assess diagnostic tests, including x-rays and laboratory tests
  • Determine, plan, and initiate appropriate treatment, adjusting as necessary
  • Prescribe medications
  • Document medical information and maintain electronic medical records
  • Educate, instruct, and counsel patients and families on health problems, treatment plans, and home care; promote preventative health care and positive health behaviors
  • Request consultations or referrals with other healthcare providers and collaborate with Kidneylink RN Care Manager
  • Promote customer service standards and develop effective relationships across all organizational levels
  • Participate in team concepts and perform duties in accordance with company policies and procedures
  • Maintain regular and reliable attendance

Required Qualifications

  • Current license to practice as an Advanced Practitioner in the state where duties are performed
  • Specialty certification
  • Two years of clinical work experience
  • Excellent written and verbal communication skills
  • Understanding of physician practice protocols, policies, and operations

Preferred Qualifications

  • Nephrology experience
