Upperline Health, the nation's largest provider dedicated to lower extremity, wound, and vascular care, is seeking a Nurse Practitioner to provide clinic-based care at our Bradenton, Florida clinic. You will work collaboratively with our local physician team to develop and implement complex plans of care for adult patients with chronic conditions such as diabetes, COPD, CHF, debility, and dementia. This role offers a regular weekday schedule with opportunities to make a meaningful impact on at-risk patients in both clinic and home settings.

Clinical Setting

You will provide care in clinic, home, and telehealth settings as appropriate. You will be part of a comprehensive interdisciplinary care team including physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers who coordinate integrated treatment addressing patients' immediate and long-term health needs.

Responsibilities

  • Develop and implement clinical plans of care for adult patients with chronic conditions
  • Gather patient history, perform physical exams, and order diagnostics in home or clinic settings
  • Develop and implement treatment plans aligned with patients' goals of care and current conditions
  • Collaborate with the interdisciplinary team to address patients' and families' physical and psychosocial needs
  • Provide compassionate care for complex patients and their families
  • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention
  • Communicate with other Upperline clinical team members and patients' community providers to coordinate optimal care and resources
  • Review patient records and care plans with Upperline's local physician on a timely basis consistent with state regulations and company standards
  • Maintain accurate patient medical records and documentation consistent with state regulations and company standards
  • Complete accurate and timely documentation according to risk adjustment standards
  • Participate in weekly interdisciplinary team meetings and scheduled education sessions
  • Provide telephonic support for escalated calls from triage nurses
  • Participate in continuing education as required by the state Board of Nursing
  • Prescribe medication as permitted by the state Board of Nursing
  • Perform other related duties as needed

Required Qualifications

  • Master's degree in nursing
  • Board certification as an Adult, Adult-Geriatric, or Family Nurse Practitioner
  • Current state registered nurse and nurse practitioner license
  • Minimum six months of nurse practitioner experience in primary care or internal medicine
  • DEA registration or eligibility
  • Basic Life Support (BLS) certification
  • Reliable automobile, valid driver's license, and minimum state-required liability auto insurance
  • Comfortable providing care in patients' homes
  • Experience with electronic medical records; AthenaHealth EMR is highly preferred
  • Proficiency with Microsoft Office products
  • Excellent oral and written communication skills

Preferred Qualifications

  • Experience managing, supervising, or coaching clinicians
  • Previous experience in geriatric care, home-based care, palliative care, or hospice care

Schedule and Travel

This is a full-time position with a regular weekday schedule. Mileage reimbursement is provided for home visits.
