DaVita Integrated Kidney Care (IKC) is seeking a full-time Nurse Practitioner to provide comprehensive, integrated care to patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). You will work autonomously and collaboratively with healthcare teams to deliver holistic care across multiple co-morbid chronic conditions, focusing on improving quality of life, reducing hospitalizations, and delaying disease progression.
You will serve patients across a designated geographic region including Stamford, South Norwalk, and New Haven in Connecticut, as well as Yonkers and White Plains in New York. You must live locally in one of these areas.
Clinical Setting
This is a hybrid-remote position combining telehealth visits with in-person work at dialysis clinics and nephrology practices within your assigned geography. You will manage complex patients through comprehensive health evaluations, medical record review, consultation with patients and families, and coordination with healthcare team members.
Responsibilities
- Perform enhanced annual wellness visits and Comprehensive Health Evaluations (CHEs) to identify and evaluate patients' co-morbid conditions and address medical, social, emotional, and financial concerns
- Manage care of CKD/ESKD patients with focus on delaying disease progression, reducing medical costs and utilization, and preventing or reducing hospitalizations
- Conduct proactive post-discharge planning and follow-up with patients at highest risk
- Exercise prescriptive authority and manage medications
- Order and interpret laboratory tests and diagnostic studies
- Collaborate with other providers and healthcare systems to coordinate quality, cost-effective care
- Assume a clinical leadership role within the healthcare team
Schedule
- Monday through Friday schedule
- Full-time, self-managed schedule
- No nights, weekends, or on-call duties
- Must be flexible to accommodate patient and nephrology partner availability
Required Qualifications
- Minimum of two (2) years' experience as a Nurse Practitioner
- Current unrestricted NP license to practice in Connecticut
- Current Drug Enforcement Administration (DEA) license or eligibility to obtain within 90 days of hire
- Current CPR or Basic Life Support (BLS) certification
- Current valid driver's license and insurance within state of residence
- Intermediate computer skills and proficiency in MS Word, Excel, and Outlook
- Demonstrated proficiency in clinical assessment and management of complex co-morbidities, including diagnosis, planning, implementation, documentation, and evaluation
- Strong clinical judgment and confidence in autonomous decision-making within established protocols and evidence-based guidelines
- Home office with high-speed internet and reliable transportation required
- Passion for delivering care to patients with complex, chronic illnesses
Licenses and Certifications

- DEA license (required or eligible to obtain within 90 days)
- CPR or BLS certification (current)
Preferred Qualifications
- Experience working in value-based care or population health, including documenting and coding from a Medicare Advantage (MA) risk perspective and conducting Health Risk Assessments on high-risk or chronically ill patients
- 5+ years' experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner, or experience in Internal Medicine, Cardiology, Nephrology, or Case Management
- Experience managing population health and hospitalization metrics
- Prior experience with Cerner or similar EHR system
Compensation and Benefits
Salary Range: $118,000 - $135,000 per year
