Nurse Practitioner (Skilled Nursing) Avir at Comfort

Avir Health Group is seeking a Nurse Practitioner to provide clinical leadership in a Skilled Nursing Facility (SNF) setting. In this role, you will work collaboratively with the facility's care team to oversee both short- and long-term residents, managing their clinical needs and coordinating comprehensive care.

Responsibilities

• Conduct patient admissions and discharges
• Perform monthly stability checks and evaluations for changes in condition
• Develop and implement advanced care planning
• Perform comprehensive patient assessments
• Prescribe treatments and manage medications
• Serve as a clinical leader within the facility care team

Required Qualifications

• Active Texas Advanced Practice Licensure (APN)
• Active DEA License
• Master's degree in Nursing
• Previous nursing or clinical experience
• Strong communication and teamwork skills
