Nurse Practitioner (Skilled Nursing) Aviata at University Hills

Aviata Health Group is seeking a Nurse Practitioner to provide clinical leadership in a Skilled Nursing Facility (SNF) setting. In this role, you will work collaboratively with the facility's care team to oversee both short- and long-term residents, managing their clinical needs and coordinating comprehensive care.

Responsibilities

• Conduct admissions and discharge evaluations
• Perform monthly stability checks and assessments for changes in resident condition
• Develop and implement advanced care plans
• Perform comprehensive clinical assessments
• Prescribe treatments and manage medications
• Serve as a clinical leader within the interdisciplinary care team

Required Qualifications

• Active Florida Advanced Practice Licensure (APN)
• Active DEA License
• Master's degree in Nursing
• Previous nursing or clinical experience
• Strong communication and teamwork skills
